Form 11:
Form 11 is used for declaration of a member of staff for both pension scheme and provident fund. This is the form which comprises the on boarding things for most organizations. This declaration form is required to be filled by the employee as part of the joining official procedure in an organization which is covered under Employees Provident Fund or EPF scheme as per the 1952 Act. Purpose of form 11 PF is for two objectives, Firstly, if the new employee or entrant was a previous member of the Employee's Provident Fund Scheme, they will carry on with a membership of the original scheme. Secondly, If the new member of staff was not a member of PF in his past job or was never been working before and his income is more than Rs 15,000 p.m. in the new job, he can elect not to contribute for EPS/EPF. Such a worker is included in the definition of expelled employee. Persons who have withdrawn their PF at a prior date or People being paid PF pension are also considered as excluded employees.

Form 11 PF:
Form 11 PF declaration form also allows the provident fund department to keep up a comprehensive database which consists of pertinent details about the employees. It also helps them considerably during inspections, cross-checking or verification of facts and audits. PF declaration form 11contains fields like Personal Information which includes Name of the member, Father’s Name or Spouse’s Name, Gender (Male, Female and Transgender), Marital Status, Contact Details including Email ID & Mobile Number; Details related to previous employer and participation in EPF and EPS; Documents required for KYC; Declaration by Present Employer etc. Every employee should have a clear understanding of what is from 11 which is covered under the EPF scheme as per the 1952 Act.
