Form 37 of ESI is a declaration by the employer regarding continuity of employment. If an employee completes 78 days of contribution towards ESI, then the employee can continue his/ her contribution whenever the employee regions new establishment. Form 37 is for carrying forward the employee IP number whenever he/ she joins the new company. The employee has to submit this form in the regional employee state insurance corporation office.
Form 37 ESI is a single-page document contains information regarding the joining of the new organization. An insured person needs to fill his/her name, employee father's name, establishment code, last date and starting date of ESIC contribution.
Form 37 is self-explanatory it is easy to fill, the first employee needs to furnish detail such as father's name and the Establishment code and address and insured persons IP number and dates of start and end of the ESI contribution. Form 37 certificate it is valid for the 9 months from the date of submission. To submit ESI form employee’s organization must be in the jurisdiction of the ESIC local office. Single condition is that insured members have to contribute his ESI share for a minimum of 78 days . if the employee desires to change his local ESI office then also he/ she must submit the form 37.
For new employees joining an organization, the employee has to submit an ESIC application form 1 i.e. (declaration form provided by the ESIC). Employees already registered for the ESIC scheme needs to submit the form 37. Form 37 is only for the employees.
