ESIC form 37 is called as reemployment or continuing employment it is a declaration by employer W.R.T employee reemployment or continuity of the employment within the time of insurable contribution that to be submitted on demand by regional/local (employee insurance corporation )ESIC office. when an employee separates from one company to join the new company to carry forward the employees ESI IP number form 37 is required to be submitted at the local office.
In order to submit form 37, the employer's office must be in the jurisdiction of the local office. Condition is that If an insured member or employee makes a contribution to ESIC for 78 days is eligible to continue their contribution if the employee willing to region in a new organization for certain period of time form 37nshould be submitted so that old ESI number doesn't overlap with new is number given by the new organisation . in certain cases this overlap might lead to rejection ESIC member claim.
Requirements to fill the form are
● Name of employer
● Address of the employer
● ESI Code of Employer.
● Name of employee
● Insurance number of the employee
● the start date of the service and current contribution period
● Date of reemployment / re-entering the insurable employment of the employee.
● Payment details for not less than of half of the No. of days worked during 31st March or 30th September.
