ESIC is a health Card provided to the member enrolled in the ESI scheme i.e. is employee state insurance scheme this scheme was implemented by the ministry labor and employment to protect workers from unexpected medical issues. any establishment employing more than 10 employees and in some states, more than 20 employees must register their employees under ESI scheme this scheme is administered by the ESIC employee state insurance corporation under ESIC ACT 1948. It is a social security health insurance scheme that provides /avails its member's medical benefits and also pension benefits for the dependant.in this scheme the employee contribution towards ESI is 4.75% from the salary and employer contributes 1.75% towards ESI considering the basic salary employee paying to the employee. employees earning more or less than 21000 rs must register to this scheme. every employee who registered with this scheme will be provided with a smart magnetic identity card also known as pehchan card or ESI card.
The details present on pehchan card/ESIC card
• the employee name,
• father’s name,
• Insurance number.
• employee's photograph
• employee's dependents photographs
Process for applying:-
ESI- employer has to address employee’s application to The Officer, Employees’ State Insurance following details
From – 01
Certificate of Incorporation if it is limited or Partnership deed if Partnership firm
Copy of Pan Card of both company and director or proprietor.
Owners Residential Address Proof & Company Address proof.
Memorandum & Articles of Association
to download ESIC card employer needs to login into ESIC portal www.ESIC.in and details of the employee should be filled in the ESIC card application form and the form can be downloaded from the same site employee should submit card family photograph containing photos of dependents to the employer. The employer has to attest submitted documents regarding registering for ESIC card after filling of ESIC card application form employee should produce it before nearest ESIC office where employee biometric scanning will be taken along with the dependent after 30 days of time period
the employee will be given 2 sets of cards,
One for employee
One for employee dependents.
If the dependents live in another location they avail the benefits by using ESI card
• Personalizes IP details on each card
• 10 digit Insurance ID
• IP Name,
• Date of Birth
• family photograph
• 10 digit Insurance number
1.Please keep this printout for future reference and bring this along with your Photo ID for all your Claim Benefits and Medical Benefits.
2.This copy should be retained with you until the Pehchan Card is received.
3.Employer to please affix employee and his family photo here and attest with an official stamp across.
* If Aadhaar number is present, this e-Pehchan Card is valid till IP is eligible for treatment.
