FORM 19

Form 19 is a for claiming provident fund final settlement, EPF scheme members who are leaving services or retiring and claiming their provident fund form 19 is used It should be submitted through www.epfindia.gov.in to EPFO and certain condition are applicable for applying form 19 those are:

1 form should be filled in block letters

2retiring after the age of 55 years

3 or retired due to incapability physically or mentally

4 retired under voluntary retirement scheme

5 settlement in abroad

6 resigned from the employment

7 discharged from the service

If the reasons are discharged or resigned the form should be applied after two months of waiting period

Required details to fill form 19 are sated below

Name of the member

Father/ husband name

Name of the company

Permanent account number (PAN)

Date of joining

And the date of leaving reason should be mentioned

PF account number

Universal account number (UAN)

Address

Bank details such as

Name of the bank

Savings bank account number

Address of the branch

Ifsc code

The application is to be submitted by the employer under whom the employee was last employed.Copy of the blank/ canceled cheque having the account number and IFS Code and Copy of Visa, Passport in case of settling in abroad the mode of payment is bank transaction and payment will be sent to the account number mentioned and transaction mostly done through NEFT. Two signatures of employee and one signature of the employer is required

FORM 10C

Form 10c is a form submitted/filled by the employee to claim the benefits under the Employee Pension Scheme (EPS) as every month provident fund contributed by the employee and employer are credited into the EPF account and if the employee wants to claim the money from his PF account Form 10c must be submitted.It should be submitted through www.epfindia.gov.in to EPFO.

Conditions for applying form 10c

1. The employee left employment before 10 years of the service

2. Employee completed 10 years of the service

3. Employee Attained 50 years of age

Names should be filled in capital letters

1. Documents to be produced

2. Copy of blank/ canceled cheque

3. Date of Birth Certificates of children of an employee for scheme certificate

4. Death Certificate in case of death of an employee

5. Succession Certificate in case of an application by the legal heir only after death

To fill form 10c following details are to submitted/filled

1. Name

2. Date of birth

3. Father’s name and/or husband’s name

4. Employer address

5. PF account number

6. Joining date with the employer

7. Reason for leaving and date of leaving

8. Full address

9. Particulars of family/nominees

10. Mode of remittance

11. Date and signature,

12. Details about age

13. Account details

14. Sum received

15. Date and signature
